Community and specialist services

This section shows you how to access community and specialist services that are provided across Sussex.

You may also find web sites relating to local hospital trusts, primary care trusts and local councils useful.

All patients must be offered a ‘key worker’ who is responsible for the co-ordination of your care. You should always be informed if your key worker is changed, and it is important that you know how to contact them.

At consultations you are welcome to ask for a taped or written permanent record of the discussion.
