Every cancer patient should have the contact details of a nominated 'key worker' who is responsible for ensuring good communication between the patient and their multi-discplinary team.
The key worker will be changed according to where your care or treatment is being provided. While treatment is provided at a hospital or specialist centre the key worker will be a member of the multi-disciplinary team (often a clinical nurse specialist).
When the patient is discharged from the hospital back to the care of the community, the clinical nurse specialist will make sure they know who their new key worker will be.
